Our comprehensive patient care program has been providing whole person care, including comorbidity management, to patients with rare and complex conditions for more than 20 years. Using real-time pharmacy and medical claims data, electronic health records (EHRs), and health plan and specialty pharmacy referrals, Accordant nurses can identify members with specialty conditions earlier, and proactively provide support to better manage their whole condition through evidence-based interventions.

Our solution for care for rare conditions is accredited by the National Committee for Quality Assurance (NCQA) in population health and case management. Our approach for rare conditions goes beyond drug therapy, counseling members on all facets of care. Together, our specialty and rare programs support a range of complex and rare conditions.

The programs are guided by our medical advisory board (MAB), comprised of more than 30 nationally recognized and specialized physicians to help ensure the latest high-quality standards of care. The MAB helps inform the development of our in-depth care management tools and clinical decision support systems based on the latest evidence-based medicine and clinical guidelines and is available to advise nurses on complex member cases.

  • Our specialty care program includes support for Crohn’s disease, cystic fibrosis, Gaucher disease, hemophilia, hereditary angioedema, multiple sclerosis, rheumatoid arthritis, systemic lupus erythematosus, and ulcerative colitis. 
  • In addition to the conditions targeted by our specialty program, our rare solution program supports amyotrophic lateral sclerosis, chronic inflammatory demyelinating polyradiculoneuropathy, dermatomyositis, human immunodeficiency virus (HIV), myasthenia gravis, Parkinson’s disease, polymyositis, scleroderma, epilepsy, sickle cell disease, and primary pulmonary arterial hypertension (PAH).

Our whole patient approach focuses on more than just the medication to include a range of challenges faced by members with complex conditions. Our nurses work to address all of a patient’s needs, wherever they are in their condition journey, to help remove barriers to care, improve outcomes and help them live their best life, as well as lower overall health care costs.
